Allina SeniorCare Transitions offers primary health care to seniors in the comfort of their home.  Our medical care team sees patients in a variety of living options including assisted living, memory care, skilled care center and transitional care at Presbyterian Homes of Bloomington.

We make house calls on select weekdays so you don’t have to travel to a clinic for appointments.  Our goal is to help you stay healthy, living as independently as possible. We can help manage medical conditions and care for you when you are sick and need medical care.
